Reporting an Event

The Report Event function is used for when an Incident, Accident, or Safety Event takes place.

To Access Event Reports:

  1. Log into the Community Portal 
  2. Locate the navigation bar on the right side of the page
  3. Locate and select "Request/Report
    1. A pop-up will generate with the different Requests/Reports your institution offers
  4. Select Report Event 
  5. Fill out the "Overview" information section

  6. Fill out all of the "Event" information section
    1. anything marked with an asterisk will be required in order to submit the EventScreen Shot 2022-09-28 at 2.21.46 PM
  7. Add data to the Report Attachments sections *if applicableScreen Shot 2022-09-28 at 2.22.06 PM
  8. Add Links *if applicableScreen Shot 2022-09-28 at 2.22.17 PM
    1. An "Add New Link" pop-up window will generate
      1. anything marked with an asterisk will be required in order to submit the EventScreen Shot 2022-09-28 at 2.23.02 PM
  9. Locate and select "Submit" at the top of the pageScreen Shot 2022-09-28 at 2.23.19 PM
